{"id":1222,"date":"2021-06-30T18:05:32","date_gmt":"2021-06-30T18:05:32","guid":{"rendered":"https:\/\/ambaccra.esteri.it\/news\/dall_ambasciata\/2021\/06\/il-nuovo-formulario-digitale-europeo-2\/"},"modified":"2021-06-30T18:05:32","modified_gmt":"2021-06-30T18:05:32","slug":"il-nuovo-formulario-digitale-europeo-2","status":"publish","type":"post","link":"https:\/\/ambaccra.esteri.it\/en\/news\/dall_ambasciata\/2021\/06\/il-nuovo-formulario-digitale-europeo-2\/","title":{"rendered":"TRAVELS TO ITALY &#8211; EUROPEAN DIGITAL PASSENGER LOCATOR FORM (DPLF)"},"content":{"rendered":"<p>All passengers wishing <a href=\"https:\/\/www.salute.gov.it\/portale\/nuovocoronavirus\/dettaglioContenutiNuovoCoronavirus.jsp?lingua=english&#038;id=5412&#038;area=nuovoCoronavirus&#038;menu=vuoto&#038;tab=5\">to travel to Italy <\/a>are required to complete <strong>the European digital Passenger Locator Form (dPLF) before entering national territory<\/strong>, following the instructions below:<\/p>\n<p>\u2022 go to the website <a href=\"https:\/\/app.euplf.eu\/#\/\">https:\/\/app.euplf.eu\/#\/<\/a><\/p>\n<p>\u2022 follow the instructions to access the dPLF <a href=\"https:\/\/www.salute.gov.it\/imgs\/C_17_pagineAree_5411_6_file.pdf\">https:\/\/www.salute.gov.it\/imgs\/C_17_pagineAree_5411_6_file.pdf<\/a><\/p>\n<p>\u2022 select \u2018Italy\u2019 as the destination country<\/p>\n<p>\u2022 register on the website by creating a personal account with username and password (required only the first time you use the website)<\/p>\n<p>\u2022 confirm the account via the link sent to the email address provided (required only the first time you use the website)<\/p>\n<p>\u2022 complete and send the dPLF following the instructions.<\/p>\n<p>Once the form has been sent, passengers receive the dPLF in PDF and as a QR Code at the email address entered during registration, which must be shown directly from a smartphone (in digital format) when boarding. Alternatively, passengers can print a copy of the dPLF to show when boarding.<\/p>\n<p>One form must be completed for each adult passenger; accompanied minors can be registered on the form of the accompanying adult. In the case of unaccompanied minors, the dPLF must be completed by the parents or legal guardian before departure.<\/p>\n<p>The digital Passenger Locator Form replaces the self-declaration.
